What insurance can and can't do.
Let's be straightforward, because this is where families most often get confused. Insurance will not cover the full cost of Forte Strong. But because we're a licensed behavioral health provider, many insurance plans will cover a portion of your son's care — sometimes a meaningful one.
How much depends entirely on your specific policy: your carrier, your plan, your out-of-network benefits, and your deductible. The only way to know what your plan will do is to check. That's what the form below is for.

A note for United Healthcare policyholders.
If your insurance is through United Healthcare, there's one extra step: United requires that you (the patient or policyholder) call them directly to authorize Forte Strong to obtain your out-of-network behavioral health benefits. Without that authorization, they won't release the information to us. If this applies to you, you'll see a note about it on the form below.
